I authorise Legado de Valor to act as intermediary in the preparation and submission of the file before the competent Ukrainian authorities, and I accept the fee model of 20% of the gross compensation amount in accordance with the Terms and Conditions, including the joint and several payment liability clause among beneficiaries. *
